Program

Memory Care is a comprehensive program designed to support individuals with a cognitive illness and the family caregivers supporting them.


Memory Care appointments are for individuals seeking initial assessment who are experiencing symptoms of memory loss or other changes in thinking. Individuals already diagnosed with any type of cognitive impairment and seeking ongoing care are also appropriate for Memory Care. Family caregivers are welcome and strongly encouraged to participate in the program.


The Memory Care Program includes services covered by some insurance plans:


  • A complete patient assessment 
  • Guidance from our team in what can be an uncertain, long journey   
  • Confirmation of diagnosis 
  • Treatment plan



Family caregiver support is available through our Caregiver Support Program, led by our experienced social worker team. This is an optional service and separate fees apply.



Schedule of Visits

Memory Care Initial Visit for New Patients

  • Approximately 50-minute video conference in the comfort of your own home with our Nurse Practitioner or Physician Assistant
  • Includes a thorough history, cognitive assessment, and review of all medications and supplements
  • Any necessary outpatient testing will be ordered during this visit
  • Dr. Laird will be involved in all Memory Care Program patients' care and will see the patients within the first three months of joining the program

Second Visit

  • Approximately 1 hour
  • Occurs 2-weeks following your initial assessment to review all findings and begin putting together your an individualized care plan
  • Copies will be provided to the patient, caregiver, and other physicians

Follow-Up Visits

  • The personalized care plan will be updated during follow-up according to your care plan and treatment schedule
