• $300 Annual Program Fee (Per Family) includes:

1. The first hour with a Social Worker experienced in Alzheimer's disease and related disorders
2. Exclusive access to the annual My Memory Care Caregiver Symposium
3. 24/7 access to the Caregiver Support website: www.navigatingagingneeds.com

• Additional Social Worker sessions can be scheduled at $100 per 60-minute session. These visits can be conducted Virtually or via Telephone calls.
